We will begin processing your application. If we have any questions you will receive a call and if we cannot reach you a letter will be sent. In the coming weeks you will receive your certificate of coverage if you purchased a dental plan. Vision Service Provider (VSP) vision plans are paperless.

When your certificate of coverage arrives in the mail, please familiarize yourself with the benefits of your program. Please review potential waiting periods, limitations, and exclusions in detail. If you have any questions, don’t hesitate to call and ask! For dental clients, if you have an upcoming procedure that may cost more than $300 please have your dentist submit a ‘pretreatment estimate’ so you can know exactly what the plan will pay.
